Step 2: Registration Application
ALL MASSAGE THERAPISTS PRACTICING IN THE STATE OF COLORADO MUST BE REGISTERED BY APRIL 1, 2009.

The following steps MUST be followed in the application process:
- Submit an original application form (photocopies and faxes are not acceptable);
- All questions must be filled out completely and accurately;
- The fee of $90 has been remitted;
- Fingerprint card (Form FD258) has been submitted to the CBI and the background check has been completed and submitted to DORA;
- The original (no photocopies or faxes) Affidavit of Eligibility form is complete; and
- Any response for follow-up information by DORA has been responded to.
Registration should take approximately 14 business days from the time that DORA receives all required information. You can check the status of your application on the DORA Web site.
It has been reported that up to 40% of all applications to DORA contain mistakes, which causes a delay in the process. Avoid common mistakes such as:
- If you have changed your name because of marriage or divorce and the name on your application is different than the name you used in school, send a copy of your marriage license or divorce documentation with the application.
- Do not omit information or lie on the screening questions. An arrest, criminal conviction or disciplinary action will not automatically disqualify you from registering.
- Do not skip any lines on the application. Have someone else double-check your application to make sure that there are no errors.
Qualifications for Registration
Until December 31, 2009, applicants can meet EITHER the education or exam requirement or demonstrate that they have practiced for 5 (five) years and have had at least 300 hours of massage education. DORA will accept an attestation for proof of education or experience during this initial registration process. This is commonly referred to as "grandfathering".
Beginning January 1, 2010, all applicants must meet both the entry-level education requirements (500 hours) and exam requirement (MBLEX or NCBTMB exam). They will also have to submit their transcripts and exam scores.
Renewal
This initial state registration will be effective for 2 (two) years. The renewal date will be January 1, 2011. There is no background check or continuing education requirement required for renewal and a fee has not yet been set by DORA. DORA will send renewal notices as a courtesy. Therefore, it is vital to keep your contact information current with DORA. However, it is each individual's responsibility to renew his or her registration.
